Independent Living Arrangements

This service provides necessary support in order for the person to remain in His/her own home and/or participate in work or community activities.

Role Summary
Is responsible for assisting the consumer in having a safe and clean home, stay healthy and clean and have good meals. This support may occur in the person’s home or in the community based on the person’s specific needs, as identified through the Individual Support Plan process.

Some of the tasks related to may include:

  • Driving/Transportation availability
  • Assisting person with emergency contact information
  • Scheduling appointments
  • Help with environmental modifications/accessibility.

Responsibilities
Employee must be prompt and reliable. If you want to take time off, must give at least two weeks notice to make other arrangements for person’s care. Responsible for asking any questions to insure person’s care is appropriate.

Help consumer pay bills; drive to and from activities and doctor’s appointments, assist with lift on wheelchair vans and any other personal care equipment.

Give verbal reminders and or prompting as directed per ISP.
Actively participate in continuing skill development activities.
Attend other training per ISP or provider request.

Qualifications
Minimum of 3 months progressive hands on personal client care experience as well as some housekeeping.

  • CPR
  • First Aid
  • Article 9 training
  • Ability to interact with families, Support Coordinator and other ISP team members.
